Health Care Professionals, Keep Your Address Updated with Your Board!

There is a legal duty on behalf of all licensed health care professionals in Florida to keep their professional boards updated with the licensee's current address.  This means that doctors, nurses, pharmacists, dentists, chiropractors, health care risk managers, physical therapists, and all other health care providers who are licensed by the Department of Health must update the DOH if they move locations.

Take a moment to go to this website and confirm what the "Address of Record" is for your license:

Now, if the "Address of Record" is not correct or is not the one you want your official correspondence coming to you at then you must contact your individual Board and make arrangements to update your contact information with the Department of Health.

Why?  It could cost you money and result in discipline of your license if you fail to do so.


Section 456.035, Florida Statutes, provides that each licensee of the Department of Health is solely responsible for notifying the Department in writing of the licensee's current mailing address and place of practice.  Failing to notify your Board or the DOH of a change of address in a timely manner can result in fines and an Administrative Complaint against your license.
